Adult registration form

This form is required for every adult working with youth. If you are not already livescanned with PAL, you will need to be livescanned May 21 at 7pm in the Viking office

Non-League play form

This form is used for friendlies or scrimmages so that the event is covered by insurance. Just fill out prior to the event.

red card sign-off form

This form is used to document the serving of a red card suspension. Have the referee sign off that the player sat out the game.


Roster Changes

If you have any roster changes - adds, releases, transfers (on or off) - you will need to surrender your official team roster (goldenrod) at the same time you submit paperwork for your roster changes.

The exception would be if you need your team roster for tournament credentials check, in which case you will need to communicate and coordinate with the Club office/League.  Make sure that you let us know the tournament date.

Instructions for adding new players for Spring 2013:

Proof of Residency

Proof of residency needs to be established:

EITHER by submitting one school document from the player's school (or SFUSD) showing the players home address (that is very important)

OR a total of 3 proofs off the list below-

Residence shall be established and supported by documents from THREE OR MORE of the following categories to determine residency of such parent(s) or guardian:
 
1. Driver’s License
2. Voter’s Registration
3. School records
4. Welfare/child care records
5. Federal records
6. State records
7. Local (municipal) records
8. Support payment records
9. Homeowner or tenant records
10. Utility bill (i.e., gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)
11. Financial (loan, credit, investments, etc.) records
12. Insurance documents
13. Medical records
14. Military records
15. Internet, cable or satellite records
16. Vehicle records
17. Employment records

Transfering or Releasing Players

Transfer A Player

  • Turn in old Player Pass
  • Transfer Form 1606 signed by releasing team coach and player parent (Note: This year, a digital signature, scan or fax from the parent will suffice, but the 1606 must have an original signature from the releasing team's coach.)
  • Original signature 1601
  • player Birth Certificate
  • $20 Transfer Fee
  • goldenrod from both teams

Release (Drop) A Player  

  • Turn in old Player Pass
  • Transfer Form 1606 signed by releasing team coach and player parent
    (Note: This year, a digital signature, scan or fax from the parent will suffice, but the 1606 must have an original signature from the releasing team's coach.)
  • Implication : Releasing a player from your team should not be done if the player intends to play for another team or in another season. Once players are released, in order to play again, they must be added back to the team they were dropped from before any transfers can take effect

 Notification of a Cal North Team Traveling to a

Non US Youth Soccer Tournament

E-Travel System
In an effort to streamline the team travel and guest player travel request process, Cal North is excited to announce the implementation of Active/League One's E-Travel and Guest Player approval system. The new system eliminates the need to submit hard copy forms into the Cal North Office. Instead, teams wishing to travel to US Soccer Federation sanctioned tournaments will be able to fill out an online form. The system will then check the submitted data against Cal North's data in Active/LeagueOne, and output a confirmation page that grants the team permission to travel provided all the information input into the online e-travel system is validated.

Additionally, if the travelling team is using guest players a similar form online is available to handle guesting players travel approval.
